Enter Spouse's info including mobile number ONLY IF the spouse is authorized to pick up child and to deal with any emergencies.
Emergency Contacts & Authorized Adults for Child Pickup
Data Entry TIME SAVER:
If you are registering more than one of your children for daycamp AND they ALL have the SAME Emergency Contacts and the SAME Authorized Adults who can pickup your children, then do the following to save time:
1) Enter in the Emergency Contacts & Adult Pickup information on the registration form for your first child.
2) On the registration forms for all your OTHER children in the field below labelled: "Same Emergency & Authorized Adults as", enter in the first & last name of your first child.
3) You can then leave the emergency contacts fields and adult pickup fields empty on the forms for all your other children.
Enter in below any additional emergency contact(s) in case the parent(s) can not be reached in an emergency.
If you would like some other Adult (18+) besides yourself or spouse to pick up your child/children provide their name(s) along with their relationship (grandparent, uncle, aunt, friend, etc.).
Parent/Guardian Permission & Release
We need your permission for the following:
Medical Permission:
1) I hereby give permission for San Francisco Bible Church Day Camp staff to administer any required medication or hospitalization deemed necessary in case of an emergency to ensure the best possible medical attention for my child.
Activities Permission:
2) I hereby give permission for my child to participate in all Day Camp activities, including field trips and walks to the neighborhood park.
Photo/Video Permission:
3) I hereby authorize San Francisco Bible Church and the Day Camp program to use any pictures and videos taken of my child for for any purpose including but not limited to future promotional use.
By checking the "Yes" boxes below you are agree to grant us the permissions described above. Checking the box is equivalent to providing your signature on this online form.
*** For your child to attend our DayCamp, parents MUST agree to the first 2 permissions in order for your child to attend and participate.
Registering for One or Two or Three Weeks & for Extended Care Option

June 19th - 23th
June 26th - 30th
Day Camp Hours: Monday-Friday 9:00AM to 4:00PM
You can sign up your child for one, two, or thee weeks.
Cost per week is $125
Extended Care (4pm to 6pm):
Cost per week is $20
Sibling Discount: For additional children in your family that are attending the same week(s) that your 'primary' child attends, we provide 25% discount off for the siblings. (Note: Primary child does does not receive this discount):
Sibling Cost per week is $93.75
Extended Care (4pm to 6pm):
Cost per week is $15
Check the weeks your child is attending:
Optional Extended Care Service (from 4pm to 6pm) is available for: $20 per child per week.
If you register your child for more than one week of DayCamp, you can choose which of the weeks they attend to add extended care. You do not have to have extended care for every week they attend.
