The Primary Parent is the parent who will be the main contact person for everything regarding DayCamp (registration issues, emergencies, etc.)
Be sure to use the SAME Email Address as you used on the Registration Forms.
"First" Child's Information
If more than one of your children are attending AND all your children are attending the same number of weeks of camp, then enter as the "First Child" the name of the oldest child.
If more than one of your children are attending BUT they are not all attending the same number of weeks, enter in the name of the child attending the MOST weeks as the "First Child".
NOW... add up the cost AND extended care cost for all your children and enter that Total cost for here:
Referral Discount Code - if you have a referral code, enter the code here:
Calculate and enter your Grand Total below.
If you have entered in a referral code then apply the 10% discount to your "Total Cost" to arrive at your Grand Total.
Otherwise enter the Total Cost above as your Grand Total
You can pay by online by Credit Card or mail in a Check.
To Pay by Credit Card
1) Select "Credit Card Payment Total"
2) Enter in the "Grand Total" Amount from above as the "Amount".
3) Enter in your credit card information.
4) Click on "Submit & Pay".
To Pay by Check:
1) Select "Will Pay by Check"
2) Then click on "SUBMIT" to complete this form.
3) Make check out to: San Francisco Bible Church
4) On Memo line enter: "DayCamp" AND Name of Your First Child listed on this form.
5) Name & Address on check must be the name and address of primary parent.
