Enter the name of the SFBC Member who is Primary Parent in the Name field below.
The Primary parent is the parent who will be the main contact person for everything regarding DayCamp (registration issues, emergencies, and whose name is being used as the referral code.)
Enter in the field below the number of children (from non-sfbc families) that are attending daycamp and used your referral code. So for example enter 8, if eight children are using your referral code.
How to use referral credits for discounts
For each child that attends DayCamp, from a family that does NOT attend SFBC AND uses your referral code on their registration form, you will receive one referral credit worth a discount of 10% that can be applied to one of your children.
Using Referral Credits & Sibling Discount:
1) Each referral credit can be applied to just one of your children to give them 10% discount.
2) You can apply more than one referral credit to a child.
3) You can combine both the sibling discount (25%) and referral credits to your 2nd, 3rd, 4th child attending daycamp.
4) However - the maximum discount that can be applied to any child is 50%. This includes sibling discount and referral credits.
For example you have 10 referrals credits.
1) You can apply 5 referral credits to your first child (so 50% off).
2) So you have 5 referrals credits remaining.
3) Your second child already gets a 25% sibling discount. So you can only use up to 2 1/2 referral credits in order to max out the discount at 50%. (You can not use 3 credits to get 55% off : 25% sibling + 30% for 3 referral codes)
4) So you have 2.5 credits remaining which can be apply to your third child.
Referral Credits are not transferable to other families.
"First" Child's Information
If more than one of your children are attending AND all your children are attending the same number of weeks of camp, then enter as the "First Child" the name of the oldest child.
If more than one of your children are attending BUT they are not all attending the same number of weeks, enter in the name of the child attending the MOST weeks as the "First Child".
Calculate Cost for First Child and enter in field below:1) Multiple $125 x number of weeks (1, 2, or 3) child is attending.
2) Multiple $20 x number of weeks (1, 2, or 3) which child is receiving extend care.
3) add the two numbers for a sub-total.
4) Apply referral credit discounts to this sub-total to arrive at Cost for First Child.
So if you are using 1 referral credit then multiply the sub-total by 0.90 (10% off).
If applying 2 referral credits then multiply by 0.80 (20% off)
If applying 3 referral credits then multiply by 0.70 (30% off).
etc.
5) the maximum discounts you can apply to first child is 5 referral code credits.
Enter Total below in "Cost for First Child"
HOW TO Calculate Cost for Second, Third and Fourth Child Using the following and enter in Cost fields below:1) Multiple $125 x number of weeks (1, 2, or 3) this child is attending.2) Multiple $20 x number of weeks (1, 2, or 3) this child is receiving extend care.3) add the two numbers for a sub-total.4) Now apply referral credit(s) AND Sibling Discount to this sub-total to arrive at Cost for this Child:
If you are using 1 referral credit then multiply the sub-total by 0.65 (25% sibling + 10% referral = 35% discount).
If applying 2 referral credits then multiply by 0.55 ( 25% sibling + 20% referral = 45% discount)
If applying 2 1/2 referral credits then multiply by 0.50 (50% discount).
5) the maximum referral credits you can apply to second, third or fourth child is 2.5 referral credits to reach the maximum 50% discount.Enter Total below in "Cost for Second/Third/Fourth Child"

NOW... add up the "Cost" fields for all your children (Cost for First Child + Cost for Second Child + Cost for Third Child + Cost for Fourth Child) and enter that Grand Total below:
You can pay by online by Credit Card or mail in a Check.

1) Select "Credit Card Payment".
2) In the "Amount" field below enter in the "Grand Total" Amount from above.
3) Enter in your name and credit card info.
4) Click on "Submit & Pay".
To Pay by Check:
1) Select "Will Pay by Check"
2) Click on Submit.

4) On Memo line enter: "DayCamp" AND Name of Your First Child listed on this form.
5) Name & Address on check must be the name and address of primary parent.
